Should we model after BCI or other areas that have already formed standards?  ISO/IEC?

1     What need does Robotic guided intervention address?

  • Neurovascular Disease? Ischemic Stroke is unmet clinical need.
  • FDA perspective? Safety and Risk-Benefit
  • Physician perspective? Balance elective/call cases, call burden, decreased radiation exposure
  • Hospital perspective? Provide emergency coverage throughout a system with focused workforce. Decreased cost.

2    Design Considerations for Robotic Systems

  • Time to set up/tear down robot and tools
  • Usability of System – Cognitive unloading of operator
  • Procedure workflow
  • Audio/Visual interaction
  • Fluiditics – Saline, contrast, aspiration
  • Feedback – Visual, haptic, force, etc.
  • Autonomous use or assistance
  • Remote specifications – Latency, data security, redundancy, etc. (warning system for inadequate connection)

3    Training and Simulation

  • Standardize training program – MD/Tech/Nurse, technique/device dependent
  • Skill maintenance program
  • Complication management
  • Flow models
  • Virtual models

4    Local, Interstate and International regulatory and licensing

  • Credentialing
  • Licensure
  • Legal liability – MD, Hospital, Robot maker, Device maker, Connection provider?
  • Procedural and Professional reimbursement
  • Requirement for neurosurgical coverage in case of a bleed etc.

5    Key Performance Indicators

  • Emergency – Time, safety, effectiveness
  • Elective – Safety, effectiveness, time

6    AI integration into NES procedures

  • Interface with EMR – track and document procedure, devices, metrics, etc.
  • Assist with procedure – catheter tracking, wire tacking , embolic deployment, etc.
  • AI-assisted image overlay/fusion software
